The Federal Aviation Administration proposed to ban within two years an older part, still used in more than 1,800 Pratt & Whitney engines on U.S. airplanes, that has shot from engines in flight at least 16 times. One such failure of the part--called a spacer--contributed to or caused the Sept. 6 crash of a Midwest Express jetliner as it was taking off from Milwaukee. All 31 people aboard were killed.
